Host Liquor Liability: Check this box if you would like the optional Host Liquor Liability coverage.
 Note: Host Liquor Liability is an optional coverage and applies when charged for; however, if alcoholic beverages will be sold at the event, are included in the price of admission, or provided at a "Fund Raiser", Statutory Liquor coverage must be purchased seperately.
Set Up / Tear Down Coverage: Check this box to indicate that you would like coverage for Set Up and Tear Down days. (Day before and Day after event) A $55 each day additional charge applies for any event desiring Set Up and Tear Down coverage.
Inflatable Amusement Device: Check this box to indicate that there will be an inflatable amusement device (i.e. Moon walks, Bouncers, etc.) in use at the event. A $55 each additional charge applies for any event employing inflatable amusement devices.
